# §5116d. Application


A grant may not be made to a [State](/usc/42/201.md?p=f) under this subchapter unless an application therefor is submitted by the [State](/usc/42/201.md?p=f) to the [Secretary](/usc/42/242q–4.md?p=2) and such application contains the types of information specified by the [Secretary](/usc/42/242q–4.md?p=2) as essential to carrying out the provisions of [section 5116a of this title](/usc/42/5116a.md), including—

- (1) a description of the lead entity that will be responsible for the administration of [funds](/usc/42/12854.md?p=3) provided under this subchapter and the oversight of [programs](/usc/42/274l–1.md?p=4) funded through the [community-based and prevention-focused programs and activities designed to strengthen and support families to prevent child abuse and neglect](/usc/42/5116h.md?p=2) which meets the requirements of [section 5116a of this title](/usc/42/5116a.md);
- (2) a description of how the [community-based and prevention-focused programs and activities designed to strengthen and support families to prevent child abuse and neglect](/usc/42/5116h.md?p=2) will operate, including how community-based [child](/usc/42/416.md?p=e) abuse and neglect prevention [programs](/usc/42/274l–1.md?p=4) provided by public and [private, nonprofit organizations](/usc/42/9902.md?p=3) will be integrated into a developing continuum of [family](/usc/42/290ff–4.md?p=d-2) centered, holistic, preventive services for [children](/usc/42/256e.md?p=g-2) and [families](/usc/42/12704.md?p=11);
- (3) a description of the [inventory](/usc/42/4370m.md?p=14) of current unmet needs and current community-based and prevention-focused [programs](/usc/42/274l–1.md?p=4) and activities to prevent [child](/usc/42/416.md?p=e) abuse and neglect, and other [family](/usc/42/290ff–4.md?p=d-2) resource services operating in the [State](/usc/42/201.md?p=f);
- (4) a budget for the development, operation, and expansion of the [community-based and prevention-focused programs and activities designed to strengthen and support families to prevent child abuse and neglect](/usc/42/5116h.md?p=2) that verifies that the [State](/usc/42/201.md?p=f) will expend in non-Federal [funds](/usc/42/12854.md?p=3) an amount equal to not less than 20 percent of the amount received under this subchapter (in cash, not in-kind) for activities under this subchapter;
- (5) an assurance that [funds](/usc/42/12854.md?p=3) received under this subchapter will supplement, not supplant, other [State](/usc/42/201.md?p=f) and local public [funds](/usc/42/12854.md?p=3) designated for the start up, maintenance, expansion, and redesign of [community-based and prevention-focused programs and activities designed to strengthen and support families to prevent child abuse and neglect](/usc/42/5116h.md?p=2);
- (6) a description of the [State](/usc/42/201.md?p=f)’s capacity to ensure the meaningful involvement of [parents](/usc/42/1396a.md?p=k-3) who are consumers, of [family](/usc/42/290ff–4.md?p=d-2) advocates, and of adult former victims of [child](/usc/42/416.md?p=e) abuse or neglect, who can provide leadership in the planning, implementation, and evaluation of the [programs](/usc/42/274l–1.md?p=4) and policy decisions of the applicant agency in accomplishing the desired outcomes for such efforts;
- (7) a description of the criteria that the entity will use to develop, or select and [fund](/usc/42/12854.md?p=3), [community-based and prevention-focused programs and activities designed to strengthen and support families to prevent child abuse and neglect](/usc/42/5116h.md?p=2) as part of network development, expansion, or enhancement;
- (8) a description of outreach activities that the entity and the community-based and prevention-focused [programs](/usc/42/274l–1.md?p=4) designed to strengthen and support [families](/usc/42/12704.md?p=11) to prevent [child](/usc/42/416.md?p=e) abuse and neglect will undertake to maximize the participation of racial and ethnic [minorities](/usc/42/299a–1.md?p=d-2), [children](/usc/42/256e.md?p=g-2) and adults with disabilities, homeless [families](/usc/42/12704.md?p=11) and those at risk of homelessness, unaccompanied [homeless youth](/usc/42/290bb–7a.md?p=c-2-D), and members of other underserved or underrepresented groups;
- (9) a plan for providing operational support, [training](/usc/42/285e–2.md?p=b-2), and technical assistance to [community-based and prevention-focused programs and activities designed to strengthen and support families to prevent child abuse and neglect](/usc/42/5116h.md?p=2) for development, operation, expansion and enhancement activities;
- (10) a description of how the applicant entity’s activities and those of the network and its members (where appropriate) will be evaluated;
- (11) a description of the actions that the applicant entity will take to advocate systemic changes in [State](/usc/42/201.md?p=f) policies, [practices](/usc/42/17061.md?p=19), procedures, and regulations to improve the delivery of [community-based and prevention-focused programs and activities designed to strengthen and support families to prevent child abuse and neglect](/usc/42/5116h.md?p=2) services to [children](/usc/42/256e.md?p=g-2) and [families](/usc/42/12704.md?p=11); and
- (12) an assurance that the applicant entity will provide the [Secretary](/usc/42/242q–4.md?p=2) with reports at such time and containing such information as the [Secretary](/usc/42/242q–4.md?p=2) may require.
